ECG of the Week 20 Jan 2014.....Questions

ECG of the Week:
Questions

20 Jan 2014

From all of the information that has been provided, I have 4 questions:
1)      What information is gained with entrainment of the tachycardia from the mid coronary sinus and the eustachian ridge?
2)      What structure does this Rhythm use to maintain its circuit?
3)      What rhythm is this?
4)      What location would be ablated in order to terminate this rhythm and restore Sinus Rhythm?

The first to answer the above questions will win a second Starbucks card....worth $10.00!!

ECG of the Week 20 Jan 2014 HINT# 2

ECG of the Week: HINT#2

20 Jan 2014

A 3D reconstruction of the right atrium with Local Activation Time was created using CARTO 3.

The resulting propagation map was created from the LAT map.
(See Below)

The propagation map reveals that right atrial activation is clockwise.

Hint for the ECG of the Week 20 Jan 2014

ECG of the Week: HINT

20 Jan 2014

From the information/hint below, can you identify the

Rhythm???

Decapolar catheters were placed in the region of the crista terminalis and in the coronary sinus. A #7 Fr 3.5mm ablation catheter was placed along the eustachian ridge.

The electrogram below was recorded with the catheters in place.

The cycle length of the tachycardia is 243mS with 3:1 A-V conduction.
----------------------------------------------------------

Entrainment of the tachycardia from mid-CS revealed a return cycle length of 286mS.

Entrainment of the tachycardia from the eustachian ridge revealed a return cycle length of 243mS

Have you figured out what this is???

ECG of the Week: 20 Jan 2014

ECG of the Week
 20 Jan 2014

What’s going on here??

**I will be posting hints about what this is throughout the week**

Once again I'm going to ask for a little more.

The correct answer will contain:
1)  The correct interpretation of the ECG
 2) An brief explanation of how you arrived with your answer.

i.e. If the ECG was 3rd degree block your answer should be something like;
“Third Degree Block. There is complete dissociation of atrial and ventricular activity.”

The first person to submit (e-mail) the correct answer and the brief explanation about the ECG will win a $5.00 Starbucks Gift card!!!
